Application for Emergency Financial Assistance
Are you a member of the Colorado Army or Air National Guard in need of emergency financial assistance? We may be able to help.
Please carefully review the following documentation and submit your application when complete. Your application will be reviewed by our approving committee and you will notified once complete. Please allow a full business week for your application to be reviewed.
Steps to a successful application:
- Download the application and fill it out completely. Please note that a signature is required from your Chain of Command. The application is set-up to accept electronic signatures for faster processing. Missing information or signatures on the application will delay application processing.
- Review the application for the required documents (listed on page 1). Missing documents will delay application processing.
- Save all documents in a file on your computer for your records.
- Email the application and all supporting documents to Lisa Cooley at lisa.m.cooley8.nfg@army.mil or call 303-905-0009 for assistance.
Steps to a successful application:
- Prior to filling out the MFRF application, please make sure you meet the following qualifications:
- Must be deployed OCONUS with a copy of your orders or on State Active-Duty (SAD) orders greater than 15 days.
- Must be a Colorado taxpayer proven by your LES statement.
- Must be a Colorado National Guard member.
- If you are unsure if you qualify for the MFRF application, you can contact your local family assistance representative (Army or Air) to further discuss your questions.
- On page one of the application, you will have one of three options to select from to identify the form of assistance you are requesting. The documentation required is listed within those three options. Please make sure you submit all required documentation with your application. Missing information or signatures on the application will delay application processing.
- Save all your documents in one file and upload to the “email completed form” on this webpage.
